Service Options Using Resources in a Community Environment (SOURCE)
The SOURCE Program (Service Options Using Resources in a Community Environment) is a Georgia Medicaid program designed to help eligible individuals remain safely in their homes and communities instead of moving into a nursing facility.
The program coordinates medical care and supportive home-based services for adults with chronic health conditions or disabilities who need assistance with daily living activities.
Services may include:
Personal care assistance
Meal preparation
Light housekeeping
Laundry
Medication reminders
Companion services
Care coordination
The goal of SOURCE is to improve quality of life while helping individuals maintain their independence in the comfort of their own homes.

Community Care Services Program (CCSP)
The Community Care Services Program (CCSP) is a Georgia Medicaid waiver program that provides home and community-based services to eligible individuals who might otherwise require nursing home care.
CCSP allows qualified participants to receive personalized care and support in the place they feel most comfortable—their own home.
Services may include:
Personal care assistance
Bathing, dressing, and grooming
Homemaker services
Meal preparation
Transportation assistance
Emergency response services
Companion care
Depending on eligibility and program guidelines, a family member may also be eligible to serve as a caregiver.

Not everyone qualifies for Medicaid programs, and that’s okay. At Hand N Hand Home Care, we also offer Private Pay Services for individuals and families seeking compassionate, professional in-home care paid for out of pocket.
Our private pay services are customized to meet your unique needs, whether you need a few hours of assistance each week or more comprehensive daily support.
